What is the procedure to make an affidavit in order to prove two names belong to the same person?

Here is the step-by-step procedure to make an affidavit to prove two names belong to same person

Step 1: Prepare affidavit draft in Word format.

Step 2: Print affidavit draft on e-stamp paper ( Buy an e-stamp paper at any co-operative bank near you. A Rs.100 stamp value would be standard for affidavit execution)

Step 3: Attach any one of the supporting documents with affidavit like aadhar, passport or driving license for lawyer cross verification.

Step 4: In front of notary lawyer, sign the affidavit and supporting documents. Must sign on all pages. Get the affidavit attested by notary lawyer.

This completes the procedure to make an affidavit to provide two names belongs to the same person.

Supporting documents should be photocopied, however, should carry the original at the time of notary, as lawyer may cross-verify the original.
