Documents required to apply for Surviving Family Members Certificate:
- Duly filled and signed application form
- Family Members Affidavit
- Death Certificate
- Family Members ID proof (Aadhaar/ Driving License/ Voter ID)

The application lead time is around 7 working days. Log into Nadakacheri website and download the certificate using your application number
